Inotropic drugs and their mechanisms of action. This report describes various old and new positive inotropic drugs with respect to their mechanisms of action. Drugs with established cardiotonic effects include cardiac glycosides, beta 1-adrenergic agents, glucagon, histamine and the methylxanthines.

This brief review has attempted to classify various old and new positive inotropic drugs according to their mechanisms of action. The major effects involved are summarized in Table 1. The inotropic properties of many of the agents discussed are related to an increase in myo­cardial cAMP levels, although these are brought about by different mechanisms.

Positive inotropic drugs have various mechanisms of action. Long-term use of cyclic adenosine monophosphate (cAMP)-dependent drugs has adverse effects on the prognosis of heart failure patients, whereas digoxin has neutral effect on mortality. There are, however, little data on the effects of intravenous inotropic drugs on the outcome of patients.

Increasing the SVR leads to increased mean arterial pressure (MAP) and increased perfusion to organs. Inotropes increase cardiac contractility which improves cardiac output (CO), aiding in maintaining MAP and perfusion to the body.

The importance of the latter mechanism is as yet not fully understood but it is conceivably of advantage because an increase in myoplasmic Ca 2+ concentration may not only lead to positive inotropic but also to positive chronotropic or arrhythmogenic effects.

The probable mechanism of action for the modest inotropic effect of digoxin is inhibition of the membrane-bound Na + /K + -ATPase pump; when this occurs, Na + increases in the cell, the exchange of Na + for Ca 2+ via the Na + /Ca 2+ exchange pump is augmented, and there is a small increase in calcium influx.
