Cobicistat is a component of three four-drug, fixed-dose combination HIV treatments. The first, elvitegravir/cobicistat/emtricitabine/tenofovir disoproxil , is marketed as Stribild and was approved by the FDA in August 2012 for use in the United States.

Cobicistat is a prescription medicine approved by the U.S. Food and Drug Administration (FDA) for use in adults and children along with the HIV medicines atazanavir (brand name Reyataz) or darunavir (brand name Prezista). When taken with atazanavir, cobicistat is FDA-approved for use in adults and children who weigh at least 77 lb (35 kg).

Cobicistat is a pharmacokinetic enhancer that has no activity against HIV. It was granted approval by the U.S. Food and Drug Administration (FDA) in August 2012 as part of a coformulation that also includes the integrase inhibitor elvitegravir and the two nucleoside/nucleotide analogues (NRTIs) tenofovir and emtricitabine.

Cobicistat is a pharmacokinetic booster that blocks the cytochrome P450 3A4 (CYP3A4) enzyme, resulting in decreased metabolism and increased concentration of many other medications. As a component of antiretroviral therapy, cobicistat is used to increase the levels of certain antiviral agents, including elvitegravir, atazanavir, and darunavir.

Cobicistat (Tybost) can be a great addition to your HIV ARV regimen, but it can't be used with all HIV ARVs. It is only approved with certain protease inhibitors: atazanavir (Reyataz) and once-daily darunavir (Prezista), NOT twice-daily darunavir. It also can't be used with many NNRTIs, like Sustiva (efavirenz).
