The N-demethyldiltiazem/diltiazem and deacetyldiltiazem/diltiazem ratios were not affected by grapefruit juice intake, which indicates that these metabolic pathways are not inhibited.

Other kinds of fruit juice besides grapefruit juice may rarely interfere with medications. For most medications, orange juice, apple juice, or grape juice can be consumed instead of grapefruit juice without any concern for an interaction. However, orange or apple juice can cause an interaction with fexofenadine (Allegra) and aliskerin (Tekturna).

Grapefruit juice interacts with many oral drugs. Compounds in the juice including bergamottin, dihydroxybergamottin, and some flavonoids such as naringin effect the activity of certain intestinal enzymes including CYP3A4 and CYP1A2. These cytochrome P450 enzymes, which metabolize many drugs, are inhibited by grapefruit juice.

Amiodarone: Grapefruit juice may decrease serum concentrations of the active metabolite(s) of amiodarone. Grapefruit juice may increase the serum concentration of amiodarone. Avoid combination.Cordarone December 2008, Libersa 2000. Amlodipine: Grapefruit juice may increase the serum concentration of amlodipine. Monitor therapy.
